Are you confused about how to keep track of your heart health? Have you or your loved one been diagnosed with heart failure or high blood pressure? Then this webinar is for you. In only 20 minutes, our colleague and pharmacist, Zana Toulany, will show you how to manage your heart health online for free using HFSA’s online tool! You will learn why it is important to manage and keep track of certain symptoms and vitals including weight, blood pressure and more.

The Heart Failure Health Storylines website (and app) was created with input from people with heart failure and congestive heart failure (CHF) to make it easier for you to manage your heart health. You or your caregiver can track your symptoms, vitals, medications, mood, and more. You can even choose to share your health summary report with your doctor or loved ones.

Here are some of the free health tools to help you manage and monitor your heart health:

  • MEDICATION TRACKER: Track your medications and get reminders to help you take your medications on time.
  • SYMPTOM TRACKER: Track symptoms and side effects to see patterns that you may need to share with your doctor.
  • SYNC A DEVICE: Import data from other health and fitness apps that you use.
  • DAILY MOODS AND JOURNAL: Track and understand your emotions and what might be driving them.
